● Order of finality (Legally Separated, Annulment) (if applicable)
● Death Certificate (Widow/Widower) (if applicable)
● Medical Certificate (If the spouse has physical/mental incapacity) (if applicable)
● Birth Certificate of the minor Child/Children
● House Sketch
● Photocopy of ITR or any proof of income (If employed)
● Specimen Signature (White background)
4. Maghulat og "reply" sa system kung walay kakulian sa imong application
5. Maghulat og sulod sa 30 ka adlaw para makuha o’ mahibaloan kung pasado na matagaan sa Solo Parent Identification Card
Note: Ang Solo Parent Identification Card valid lamang sa usa ka tuig apan renewable kini.
